The "5 Whys" Technique

The "5 Whys" technique is a useful tool for identifying the root cause of a problem.

The "5 Whys" Method

By repeatedly asking "why" something happened or exists, you can uncover the underlying cause of the issue.

As the illustrations below indicate, you can get to the root cause of almost any problem by asking "why" five times. When we ask why a particular event or problem (effect) occurred, we discover an underlying cause. Repeat the process at least five times, you're probably going to be describing a deep root cause or system weakness.
