Interactive Case Study Activity

On October 10, 2025, a catastrophic explosion occurred at the Accurate Energetic Systems, LLC (AES) explosives manufacturing facility in McEwen, Tennessee, resulting in 16 fatalities, multiple serious injuries, and extensive damage to the site. The blast was so powerful that it was reportedly felt more than 20 miles away. At the time of the incident, approximately 24,600 pounds of explosive material were present inside the building, with an estimated 23,000 pounds consumed in a series of detonations. In the aftermath, about 1,000 pounds of unexploded energetic material were later recovered and safely disposed of through controlled burning. On October 27, 2025, the U.S. Chemical Safety and Hazard Investigation Board (CSB) formally opened an investigation to determine the cause of this devastating event. The investigation is ongoing as of May 2026.
