Exam Information

Final Course Exam

After studying the course material and completing the knowledge check activities, students can take the course exam. The course exam is designed to test the students understanding of the course material. Course exams are composed of multiple choice questions selected from our extensive question bank.

Exam Characteristics
  • Each course final exam consists of 15 questions randomly selected from a question bank.
  • You have two (2) hours to complete the course exam.
  • The exam is open book. You can use course modules, review notes, or the course study guide to help answer questions.
  • You must achieve a minimum score of 70% to pass the exam.
  • You may retake the final exam if you fail to pass it:
    • You may take an exam three (3) times within a 48-hour period.
    • After the third attempt, students must wait a period of time before attempting the exam again. During this time, you should review the course material to improve your understanding.
  • After each exam, you receive an email with information on missed questions. We encourage you to review the course material to improve your understanding.
  • When you pass the final exam, you may not retake the exam to raise your score.

Our forklift safety courses are designed for forklift operators, employees who work around forklifts, competent persons responsible for evaluating hazards and performance, and managers who oversee forklift safety programs.

For employees who will operate forklifts, OSHA requires formal instruction, practical training, and a performance evaluation. Operators must be at least 18 years old to legally operate powered industrial trucks. Our courses cover a range of forklift types to support diverse workplace requirements:

  • Class I: Electric motor rider trucks
  • Class II: Electric motor narrow aisle trucks
  • Class III: Electric motor hand trucks or hand/rider trucks
  • Class IV: Internal combustion engine trucks (solid/cushion tires)
  • Class V: Internal combustion engine trucks (pneumatic tires)
  • Class VI: Electric and internal combustion engine tractors
  • Class VII: Rough terrain forklift trucks

These courses are applicable across construction, general industry, maritime, and agricultural settings unless exempt by applicable OSHA standards. They support role-based training needs from basic awareness to operator qualification, competent person development, and program-level oversight.

To earn your OSHA forklift operator certificate, you must complete Course 620 OSHA Forklift Certification and pass a comprehensive final exam. You have an unlimited number of attempts to pass the final exam.

You have 120 minutes to complete the final exam, which includes 20 questions and covers all course topics. You must score at least 70% to pass the exam.

Upon passing the final exam, you'll need hands-on training and a workplace performance evaluation, as mandated by OSHA, to obtain your official forklift license. OSHAcademy provides a Forklift Operator Evaluation Form for your employer to assess your proficiency. Evaluation areas include:

  • Traveling
  • Picking up a load
  • Putting down a load
  • Parking
  • Fueling and battery recharging
  • Forklift inspections

After passing the evaluation and completing necessary paperwork, you'll be officially forklift certified by your employer, in line with OSHA certification standards. Certification is valid for three years, after which you must undergo another performance evaluation. OSHAcademy advises all forklift operators to complete refresher training when certification expires.
